We accept the following insurances:

We do not accept Aetna, Kaiser, Medicaid, or Medicare at this time.

If you have a different health insurance than the ones listed above, we are considered an out-of-network provider and you are subject to our self-pay prices.

 

We do not bill out-of-network claims. 

 

However, we will be happy to supply you with documentation to file an out-of-network claim with your insurance. To find out how our services could be reimbursed by your insurance company, you will need to contact them directly. We recommend asking your insurance company the following questions:

 

• Can I be reimbursed for mental health therapy sessions/medication management by an out-of-network provider?

• What are my out-of-network benefits?

• Is each session reimbursed at an equal rate, or am I reimbursed more or less over time?

• Is there a limit on how many sessions I can be reimbursed for over a certain period of time or over the life of my insurance plan?

• Does my provider have to provide me with a diagnosis in order for me to submit my claim for reimbursement?

**We charge self-pay rates for couple's therapy sessions. 

 

While we do accept several insurances, we charge our self-pay rate for couple's counseling. Why do we do this?

Insurance claim = CPT code + ICD-10 diagnosis code​

 

The Current Procedural Terminology code associated with "couples/family counseling" (90847) is a covered service under most insurance plans. In fact, if you call your insurance company, they may tell you that you have coverage. However, the diagnosis code appropriate to most couple's counseling services (Z63.0) is not considered medically necessary by insurance companies (even if we disagree). So, while the CPT code may be approved, the other half of the equation is not, equaling a non-covered service. To stay compliant with insurance contracts, we will bill in-network insurances for these services. However, please be advised it will likely be denied and insurance will indicate the client is responsible for the full rate. Thus, we charge the self-pay rate as indicated on our website.

Payment:

We accept cash, check, debit/credit cards (Visa, Mastercard, Discover, & AmEx), and HSA/FSA.

 

Checks can be made payable to:

"Cloud Break Therapy, LLC". There is a $30 fee for a returned check.

 

Cancellations:

Be advised, however, there is a $120.00 cancellation fee if you cancel without a 24 hour notice. Feel free to talk to your provider; We understand illness, jobs, child care and weather cause us all difficulty at times, but please be sensitive and aware that your provider's time is as equally important as yours.

 

Emergencies:

We do check our voicemails and emails regularly. However, if you experience a mental health emergency, please call 911, or go to your local hospital or emergency clinic and give them your provider's number. All releases of information can be faxed to (703) 289-0385 or emailed directly to your provider.
